Insurance
CCC clinicians are considered in-network providers for Blue Cross Blue Shield (BCBS) and Blue Choice PPO Plans, Aetna, Cigna and The University of Chicago Student Health (U-SHIP). We are not in network with BCBS HMO plans or the BCBS MyBlue Plus plan.
We will bill your insurance directly for in network plans. For all other plans we are considered out of network. We can provide you with an invoice to submit to your insurance company for possible out of network reimbursement.
Below are some questions to ask your insurance company to get more information about your copay, etc. There is usually a number on the back of your card that you can call to get this information.

Do I have mental health benefits?
What is my copay/coverage amount for outpatient therapy session?
Do I have an annual deductible and do mental health services apply toward my deductible?
How much of my annual deductible has been met this year?
How many sessions per year does my health insurance cover?
Is approval required from my primary care physician?

Cancellation Policy
We ask that you cancel your appointment with at least 24 hours notice. Unfortunately, due to our inability to utilize that time, we will charge the full cost of the session and we are unable to bill insurance for missed appointments.​
